I have a claim that is being denied on the IOL master for "preoperative visit was included in the payment for the surgical procedure". The provider completed the IOL Master the day prior to a complex cataract extraction. Is there a new rule about this? This is the first time I've seen a denial for this, or is it because it is the day prior they will not pay?
