I am reading conflicting information. Can advanced care planning (99497) be billed with the IPPE (G0402)? I am reading that the patient costs is not waived when billed with IPPE but it is waived with the AWV. I understand time has to be documented to be able to bill 99497.
 
I also have a question in regards to Advanced Care Planning for traditional Medicare. My providers are telling the to use the 1158F code which I didn't think traditional Medicare uses the CPT II codes. I looked it up on the Medicare website and I'm seeing that it is to be billed with 99497. Am I correct?
