Wiki Is E&M billable prior to portacath?

What are the opinions about billing an E&M for a pt that was scheduled for a portacath placement. The Onc office referred a new pt, we didn't see them in the office, we just scheduled the port to be placed. Can an E&M be billed with my surgical code? Can the Mod 25 be used in this senario? HELP... Thanks.
 
You didn't see patient before taking to OR?

So you took the patient straight to the OR without seeing him/her?

Every procedure includes as part of the reimbursement the appropriate pre-operative evaluation.

If all that was done was the limited exam that would be inclusive to placing the port, then all you code is the procedure.
