Below is the progress note. The patient is seen in the hospital by MD & fellow. Code given by provider is 99231. (My thought is that this is not billable as an e/m but unsure.)

Tracheostomy tube exchange
6 shiley cuffed and 6 shiley uncuffed tracheostomy tube placed without incident
Inner cannula from new tube placed in baggie on wall above patient's bed
Will place orders for capping tube

Patient seen, personally evaluated and examined and all/key clinical findings confirmed, plan discussed and implemented. Agree with notes as amended above. (this is the attestation of the MD)


If not billable as an e/m is there a code I can bill for trach change?
