CPT 11626, 15220, 15221, 20245 were billed for the below op note. There is question regarding 20245 being billable, coder and provider disagree with the bone removal being included in the margins, and what the correct coding would be.

With limited information, is this area of the scalp being coded? The cpt codes provided do not match the scalp, 11626 would be the only code that has margins ,as it is dealing with a skin lesion. Once you get to the bone you code the 2xxxx codes . I would code both the 116xx code and the correct 2xxxx code as it's two separate areas. I can't tell by this op note as it does not seem complete. But you should have 2 dxs in pathology. Hope this helps
