I have an US report on a pregnant woman that only documents the following elements: Placenta location, Presentation, Est. fetal wgt, Gestational age (27 weeks, 4 days) and EDD. It then states "documented structures are free of abnormalities". The Radiologist coded this a 76811. Wouldn't it more appropriately be a 76815, since most of the required elements aren't reported on and there is no description of why the remaining elements weren't evaluated??
