Can anyone tell me if there are LCD or NCD's for CPTIII codes? I am trying to find medical necessity guidlines for iStent 0191T, can't find anything anywhere.

There is . Please email me directly offline and I can send them to you. It has changed in 2014 for physicians and ASC. The codes are different for each setting.
