Does anybody know what CPT code I should use for an IT band tenodesis?
The physician suggested 27427, which is for Ligamentous reconstruction (augmentation), knee; extra-articular, but I'm not sure if this is correct. The other closest code I was able to find was 27305, for Fasciotomy, iliotibial (tenotomy), open.

I agree with your doctor. In AAPC coder, 27427 reads "He then repairs the ligament by suturing the torn ends together and by attaching the ends to the bone using screws, staples, etc. Next, he may use a graft, which is a strip of tendon usually taken from a patellar tendon or hamstring muscle, to reinforce the stability of the ligament and for anchoring the ligament satisfactorily with the bone."
A tenotomy cuts the tendon and a tenodesis attaches the tendon to the bone.
