This question might sound a bit strange but there is a bit of debate over the correct answer. We all know that for IV's, the start and stop time along with the substance administered must be documented. What about the site of the IV? Is there anywhere at all that states the site must be documented. (aside from common sense) If there is a guideline somewhere, would love to see it. Thank you.
