Does an IV blow out /infiltration where another IV site access is restarted in a different area constitute another "initial" (left hand) when there was already an initial (right hand) prior?

According to CPT and CMS guidelines for IV infusion and injection coding (96360–96376, 96365–96368):
  • The term “initial” refers to the first IV access site that’s used to start the infusion or injection encounternot each physical IV start.
  • Even if the first IV infiltrates or fails, and you must restart the line at a different site, it does not create a new “initial” service.
  • You continue to bill the same infusion sequence as a continuation, not as a new “initial.”
  • Documentation should note that the IV was restarted due to infiltration, but the same infusion service continues
