Good Afternoon:

I am having an issue finding ICD-9 codes that support medical necessity on the Highmark website. Are these mandated by CMS or Carrier? I am still able to pull them off of the Trailblazer website, but they may vary if the carrier mandates. If anyone can help me out I would appreciate it.
