How do I know when to use EA, EB, or EC modifier with the diagnosis. Is there a listing of diagnosis that are used with each modifier listed somewhere?
 
EA- ESA, Anemia, Chemo induced (covered when medical necessity met)
EB- ESA, Anemia, Radioinduced (never covered)
EC- ESA, Anemia, Non Chemo/Radio Induced (covered when medical necessity met)

See CMS Transmittals 1412 & 1413, in addition to the LCD for ESA
