Hi,
I am looking for a HCPCS code for Hydroxyprogesterone. J1741 was deleted in 2001. Does any one know what the current code would be?

From my understanding this code can not be submitted under medical claims only pharmacy would bill for this, Otherwise it would be rejected.
 
J3490, but you need the NDC number. I am having trouble finding the correct number since this is a compounded drug.
