Good Afternoon... I am confusing myself and need help. How do you format the NDC code info needed for J1885 in box 24 shaded area above CPT J1885 on the claim? The medication comes in a vial of 60mg/2ml.. I know J1885 dosing units are 15MG per unit..so 2 units equals 30mg and would be 1ml and 4 units would be 60mg or 2ml... I am confused how I write this in the box.....would it just be N400000000000 ML1....because the 11 digits are for a specific drug so they should let the claims processor know 2 units is equal to 1ml and 4 units would be equal to 2 ml.... does that make sense? Any insight would be helpful. Thank you!
