I do risk adjustment coding from home assessments done by providers who are not PCP, sometimes it is difficult to query them. One assessment in particular has me confused. The provider assigned these codes with the ICD10 attached in the same assessment for the same patient with the same medications listed, J42 unspecified chronic bronchitis and J45.909 unspecified asthma, uncomplicated. I was told that these 2 codes convert to J44.9 COPD, unspecified, however, the code book does not direct to this code for both these codes, there is an excludes 1 for J44. codes listed under both of these. How should this be coded/interpreted? Thank you!
