New to Opthamology--Retina specialty. Currently group is submitting unlisted J3490 for Avastin 0.05ml. Should they be using J9035 instead? Any feedback is greatly appreciated.

Need to check with your carrier to see how they want it billed; our carrier wants us to use HCPCS code J9035 for date of service on or after 05/01/2010.
