I wanted to make sure that I'm actually going to billing Medicare Part B correctly and I need someone's opinion. For ex, a patient came in for IV chemo Velcade and patient received an actual 1800 mcg and wasted 1700. Now, I think I will bill J9041 with 18 total units and then 2nd line another J9041 with modifer JW and total units will be 17. Did I get that right? Need help please.
