Does anyone have information regarding billing guidelines and/or covered diagnoisis for the captioned drug. We would be using this for bladder cx dx 188.9.

alimta is listed as 2A for 2nd line single agent metastatic for dx 188.0-188.9. Medicare would cover this dx as long as the pt mets this criteria.
