How do you code medication given to a patient that is less than the standard/normal mg given?

Example -J0696 Rocephin 250 mg is listed yet the order is for 125 mg? Do I use the 52 modifier or?????

I have posted this and have had 39 reviews however no one has an answer. Does anyone know the answer or where I may find the answer?

We do not use any modifier. You can use unit 1.I read/ heard from someone earlier once you open the single vial you can not reuse . So you can bill J 0969 X1.

Regarding chemo drugs if the nurse mentioned what was used and what was wasted you can bill different.
