Hi, I just started a job for a billing company and we have many accounts all over the states. I am working on billing for an OB-GYN in Washington State, she billed for a 99202, 90649 and 90471. My dx code for the 99202 was 346.40, 625.3 and 626.2 with modifier 25for the vaccine I used V04.89. Can any one help me with why Washington would not be paying for the new pt visit and the vaccine. They only paid for the admin. Thanks
