1)The doctor injected both knees with Kenalog 40mg using ultrasound, could you help me code? 20611 but what modifier if both knees were injected, I will use J3301 for the Kenalog.

20611-50 should work for MCR and those plans that follow their guidelines. Commercial payers might require two separate line items with LT and RT modifiers.
