My physician did a RT superior/lateral genicular, RT superior/Medical Genicular and FR inferior/Medical Genicular nerve RFA. He then did the exact same procedure on the LT side. How do I bill this out?
 
I believe you can report CPT 64640-50 64640-50 64640-50 if these are considered separate nerves and there is individual treatment of each nerve.

I am not real familiar with the anatomy being described, but CPT 64640 seems like it is appropriate for this service if it is non-pulsed.
