I'm looking for some help trying to find information about what is considered appropriate documentation for usage of the KX modifier for PPM implants.
Is a Dx in an Op report only considered sufficient per CMS? Or do we need documentation in an H&P also?
Seems the only thing I can find is related to DME from 2010, nothing specific to the PPM implants.
