When coding T-12 and a L-4 kyphoplasty, would you bill 22513 and 22514, both primary codes?? or 22514 or 22513 with the 22515 add on level?
 
use one primary procedure code and an add on code for additional levels even when crossing spinal regions (from my AANS book)
