Wiki Lab Modifiers

When would 59 v 91 be appropriate, and does Medicare accept the X Modifiers on Lab charges. I am coding these on the hospital out patient side.

I work in lab billing and we find that the 59 is used on professional charges but that MC accepts the 91 on everything else lab related. We do not use the X modifier
