I have 2 questions, similar in nature, but for different tests.

1. family physician ordered a lab test for Vitamin D testing(82306) with a DX of Leg cramps(729.82). This test generally has a trouble getting paid if it is not linked with a screening for osteoporosis or with Diagnoses of a>vitamin D deficiency or b>osteo....
My question is : if the lab test comes back before we do the billing and it is +ve for vitamin d deficiency, can we link it with the DX of (268.9)? and not the leg cramps?

2. a lab test for testosterone total(84403) was ordered with a physical with a DX of ED(607.84). That is another DX that has trouble getting paid (not showing medical necessity according to the payors). There is hypogonadism(257.2) that gets paid most of the times with this test.
My question is : if the lab test comes back before we do the billing and it is +ve for low testosterone, can we link it with the DX of (257.2)? and not the ED?
