Hi, What documentation requirements are need in order to bill for labs in office? It seems like an odd question but we're finding that we usually only have results not how the specimen was obtain (CPT 87426, 86328) or how it was detected (CPT 87880, 87430). Any resource would be greatly appreciated as we'll need to supply it to our providers.
