We have some surgical questions regarding Laminectomies and what would be correct coding. Our questions are regarding coding the 63047,63048 vs 63030,63035.

We would appreciate any insight.

1) L5-S1 Bilateral Laminectomy w/facetecomy & foraminotomy, Diskectomy - Diagnosis / Radiculopathy, Herniated Disk
2) L4,L5,S1 posterior Lumbar Laminectomy w/bilateral facetectomy & foraminotomy, left L5-S1 diskectomy, right L4-L5 disckectomy ? Diagnosis / Lumbar Stenosis and Radiculopathy
3) Right L5-S1 posterior lumbar laminectomy, facetectomy & foraminotomy with microscope ? Diagnosis / Right L5-S1 foraminal narrowing and right L5-S1 radiculopathy
4) L3-L4 laminectomy w/ facetectomy & foraminotomy ? Diagnosis / Left lumbar radiculopathy.
