My surgeon is doing a TLIF above and below (an already fused level done with a cage) and he is doing lami’s for decompression at all levels. My question is am I able to bill the new lami code 63053 at the level that was already fused with the interbody?

OPERATIVE PROCEDURE:
1. Laminectomy, L3 and L5, for decompression at areas of stenosis at levels with interbody fusions performed.
2. Laminectomy, L4, for decompression previously fused area.
3. A 360-degree left-sided transforaminal lumbar interbody fusion with PEEK cage, L3-4.
4. A 360-degree posterolateral interbody fusion with PEEK cages, L5-S1.
5. Pedicle screw fixation, L3 through S1 (Alphatec Arsenal screws).
6. Locally harvested autograft supplemented with bone morrow aspirate, Mosaic, and allograft chips.

63053 is an add on code for 63052

63052 as long as documentation is there to support it.
From the 2022 CPT changes book, "Meaning more extensive decompression beyond the typical dissection needed to complete the interbody arthrodesis approach and intervention."
These codes must be used in conjunction with 22630, 22632, 22633, or 22634
"Note-this is an add on service. Only consider the additional work related to bilateral laminectomy with decompression of the cauda equina and/or nerve roots."
