Our doctor did a L4-L5 bilateral laminotomy for lumbar radiculopathy & lumbar stenosis. No disk work was performed. Should this be coded 63030-50 or 63047 & 63048?

Any help would be appreciated.
 
If he did a facectectomy and foraminotomy I would be 63047 and 63048 assuming that the dr dictated where he performed the foraminotomy and/or specified nerve roots decompressed. If he did not specify nerve roots decompressed and/or where the foramina was performed it would be 63047 once no additional level code. If he does not dictate the facetectomy/foraminotomy I would look at code 63005.
