I have a case where they started out doing a lap but there were to many adhesions so they converted to a total abdominal hysterectomy this was after they spent 40 mins with lysis of adhesions. Can bill for the lysis of adhesions since it took that long or can I only bill the 58150?

I belive you can only code the open. But certainly use -22 and include as one of your diagnoses V64.41. Hopefully your doctor documented the 40 minutes of lysis of adhesions.
