Doc did a Lap. lysis of adhesions and a lap. placement of tunneled PD cath 49324 with the dx of Renal failure, need for dialysis, diabetes and HTN, lysis of adhesions.

My ? is what would be the primary dx for the 49324?

And sinse the Lap. lysis of adhesions is bundled with 49324 would it be appropriate to append a 22 for all the time spent on lysis of adhesions?

If the doctor documented that the amound of time the lysis of adhesion extended the normal operation time, modfiier 22 would be appropriate. I would use the renal failure as the primary dx.
