A patient had a laparoscopic left inguinal hernia repair followed by a laparoscopic umbilical hernia repair. Can you code both 49650 and 49652?
After the inguinal hernia repair was completed "the pneumopreperitoneum was deflated. I placed a 5 mm port inside the abdomen under direct visualization. There was a small umbilical hernia which was identified. There was a piece of preperitoneal fat which was removed from the umbilical opening. Once this was cleaned, then a 9 cm Parietex mesh was placed into the abdomen. It was tacked to the fascia circumferentially and then four sutures were placed at the 12, 3, 6 and 9 o'clock positions utilizing Endoclose. We utilized an 0 PDS suture in a U-stitch type fashion. Once this was completed, the abdomen was deflated. The skin incisions were closed...
A new incision was not made, but the port was inserted into a different cavity for the umbilical hernia repair. Is the umbilical hernia bundled or separately billable? Thanks for any help!
