Hello,
I'm not sure if this should be coded as a revision. Pt has 2 yr hx of gastric sleeve with recurrent hiatal hernia, which is causing her problems. Now my doc will do a laparoscopic roux en y bypass, and repair of hiatal hernia. Is this considered a revision? The sleeve itself is not being revised, so I don't know if I can use the 43644, along with the hiatal hernia repair, or if it is still considered a revision of the bariatric surgery.
Thanks for any input- I'm new at bariatrics!
 
Personally, I think it's a 43644, possibly with a -22 modifier if the previous surgery makes it significantly more complicated than the norm, & if your doc documents that, plus the hernia repair. We go around about that a lot here, too. I agree with you that it isn't a revision if they aren't actually revising the prior procedure.
