Hello, I have a provider that did a Diagnostic Laparoscopy, Laparoscopic lysis of adhesion, and laparoscopic suture ligation of the bleeding area (left lumbar trocar region). Can we bill 49320, 58660, 37617? I cant really find a code that relates to suture

1) "Bleeding area". Was this occurring prior to surgery and the surgery was to repair this? If not, you would not code for controlling bleeding during the surgeon's own procedure
2) 37617 is specifically an OPEN procedure. I am not aware of a laparoscopic equivalent, but not my area of expertise. IF it is billable, and there is no laparoscopic code, you would bill unlisted.
3) 49320 and 58660 are NCCI bundled and cannot be billed together.

You can't bill a diagnostic laparoscopy and a procedure via the laparoscope as the diagnostic part is inherent in the procedural surgery. Hence, 49320 and 58660 will always be bundled. I agree that 37617 is an open procedure so would not apply here and if the bleeding was noted as a result of the lysis the suture ligation would be included. If the patient had the problem at the start of the procedure and was one of the reasons for doing the diagnostic procedure you can add a modifier -22 to 58660 if the documentation shows significant additional work.
