I'm hoping to get some opinions/advice regarding this Hospital Outpatient surgery:

This case was a laparoscopy converted to laparotomy for ectopic pregnancy with removal of RT-fallopian tube. During the laparatomy incision, the surgeon was concerned they might have "nicked" the small intestine. They then increased the incision length and proceeded to remove 3 ft of the ileum to inspect for any possible damage. Nothing was found and the intestines were replaced and the procedure carried out without any further complication.

I know i can't use Mod -22 since this a Hospital setting, so i'm stuck on how to charge for the extra time spent on this. What would the consensus be on a complication ICD-9 code and a "open exploratory intestinal" CPT code? This is a commercial insurance patient.

Any advice would be appreciated
