Any one familiar with coding Laser CPT 66761. Trying to figure out what are the documentation elements you need for this procedure. Here is what our provider is doing thus far, but need to verification if correct. Can anyone HELP!!

Laser:　　YAG PI
Eye:　Both eyes.


Number of shots:　OD: 　3 pulse x 5 　　OS 3 pulse x 6
Power:　7.0 mj
Size:

Notes:　RTC as scheduled
 
None of our doctors do both eyes on the same day. Generally insurance won't pay for both...at lease I've never seen both paid. Same goes for injections.
As far as I can see that's about all our doctors put as well. I'll have to look again when I come across another Yag
