A recent CMS target probe on our AWVs identified missing elements in the HRA. Providers have asked if we can query the patient subsequent to the encounter and the American Family Practice group has stated no, this would be fraudulent. Any documentation experts who can weigh in and provide CMS reference we could use in an audit?
