I hope to get some feedback on my questions:

Long term care facility documentation- When a pertinent entry was missed or not written in a timely manner, a late entry is used to record the information in the medical record.

To clarify late entry- patient was seen today- entry documented tomorrow or as later than 1-10 days.

My question is - Will this service be billed (inpatient facility E/M code codes)? Primary Insurance is Medicare----- If yes or no - what's my resource? Any link or article address will be appreciated.



Second part of my question is transcription backlog--- notes are dictated on the date of service, transcribed and returned to doctor as late as 2 weeks. Can these notes be billed???
