If a patient comes in with history of smoking do we use the diagnosis Z87.891 with F17.211. If we don't use F17.211 what are the guidelines for using this code because obviously it's for a past smoker who is in remission which is confusing with past history of smoking Z87.891. We have a coder coding both for both of the above codes and I need to get this cleared up fast.
