We had a new pt come in, no leap exam was done. They came in a couple of weeks later and had a leap exam done. We billed the g0245 for the inital evaluation. The insurance is denying for a new pt code. Is that right? How can we bill for a g046 follow up if this is the first time the leap exam was done??? Please help
