When he excises a sebaceous cyst and closes the subq &/or fascial layer(s) separately, you use the integumentary excision codes WITH the intermediate repair codes. This is also true of other lesions primarily involving the skin, such as nevi, keratoses, skin cancers, etc.
When he excises a lipoma, sarcoma, or other tumor primarily involving the subq, or the fascia, muscles, or other connective tissues, then you use the musculoskeletal system excision codes. Either simple OR intermediate closure is included in these codes. There is quite a bit of guidance in the CPT book in the sections immediately preceding these groups of codes.
