Patient came in for neoplasm of uncertain behavior and sk on back. Doctor did cryotherapy on sk and biopsy both which was sent to pathology. Can I code 11120 and 11170 together. Pathology report states shave biopsy d48.5 and L82.1
Assessment
1. Neoplasm of uncertain behavior (D48.9)
One consistent with SK, will biopsy for reassurance considering size and color. Second
biopsy complete due to black color with abnormal borders.
2. Seborrheic keratoses (L82.1)
cryo x 1 to upper back large SK. Pt tolerated well. Will refer to derm for full body check and
to discuss the SK on side of his face in hair line.
3. Facial skin lesion (L98.9)
see above.
Pathology Report
SPECIMEN SITE : RIGHT UPPER BACK, LEFT UPPER BACK
Procedure
With patient verbal consent, The mole and surround skin is cleansed with alcohol swab. Anesthetized with 1 percent lidocaine with epinephrine. A dermablade is used to clear mole from skin. Antibiotic ointment applied to area, bandage applied for dressing, minimal blood loss. Pt tolerated well.
Cryosurgery
Liquid Nitrogen applied to x 1 lesion.
Seborrheic Keratosis to upper back
Patient tolerated the procedure well. Clean with Hydrogen Peroxide until healed.
