Assessment and Plan
(1) Tinnitus, bilateral:
Plan:
Discussed differential for tinnitus (hearing loss, muscular tension, middle ear dysfunction, anxiety). Recommend tinnitus masking.
(2) Endolymphatic hydrops of right ear:
Plan:
Discussed previous low frequency sensorineural asymmetry on right and resolution of said hearing loss with resolution of symptoms of aural fullness are very consistent with endolymphatic hydrops. Again reviewed recommendations of low salt diet (also discussed other potential triggers including alcohol, caffeine, chocolate). Discussed that stress can certainly be a trigger for this condition. He relates that he will be moving to Italy shortly for a new job. Copies of audiogram tests given to patient. Recommended that he establish with ENT there for continued monitoring/ongoing care, and follow up for repeat audiogram testing if fluctuations again noted.
