Wiki Level 3 or 4 for Preop

Assuming there is no Time factor for counseling/coordination of care how would we score the following?
A preop is done for a ganglion cyst removal so there's just one diagnosis.
History and Exam sections score a level 4 easily.
What would make this a level 4? (regarding MDM)
