When billing an E/M with a procedure, can we use the diagnoses for the procedure to determine complexity and risk? I know I can't for data. For example the patient came in to the dermatology clinic for a full body skin check. The provider is informed of a changing mole, and two AKs are destroyed with liquid nitrogen. I would bill 11102 and 17000 & 17003. The additional diagnoses are Seborrheic Keratoses and history of non-melanoma skin cancer. Would the dysplastic mole and AKs be able to be counted for complexity, and the choice to do biopsy and AK destruction be included in risk? Or would I only use the SK and history of skin cancer diagnoses for the E/M portion?

I hope I explained that clearly.
