Can V58.67 be coded with a diabetes code of 240.40, 240.60, etc?

Yes, as long as it is documented that the patient is taking insulin. You would only use code V58.67 on dates of service prior to Oct. 1. For dates of service Oct 1 and forward, you would use Z79.4.
