Can someone direct me to information regarding linking a seperate "Problems List or Active Problems List" that is found seperately in the chart from the providers dictated note for any given date of service. I have a a note where the dx codes are linked by number ( #1,2,3, etc...) in correlation with the "Problem List" in the providers Assessment. This is pertaining to Medicare Patients. I want to add the date and signature is not on the problems list to connect the two for any DOS- or Service dates being connected.--Thanks in advance for a point in the right direction !:D Michelle
