Hi all,
When a lipoma is dissected DOWN TO the fascia do you code from the integumentary or musculoskeletal?
Procedure note
Procedure: Shoulder subcutaneous lipoma excision
6cm lipoma of shoulder dissected down to the fascia. Potential space was closed using 3-0 vicryl sutures. Skin was then closed & reapproximated using 4-0 prolene total of 4 horizontal mattress sutures in a non-layered fashion.
Code: 11406, or 23071, or 23073?
Repair code for closing potential space/skin?
I appreciate any & all feedback.
 
If they dissect down to the fascia, they are not entering the fascia. The fascia is where they stop indicating they only entered the subcutaneous level. All codes 20000 and above have the closure of the surgical wound bundled into the procedure.

You would only code the closure if a 10000 code from the Integumentary System is coded.

In this case, 23071 is the correct code. Lipomas are mostly found in the subcutaneous.
