Patient had a repair of multiple laparoscopic incisional herina with transfascial sutures and perimeter tacks, with extensive lysis of adhesions. I came up with cpt code 49655. Is there a code for the lysis of adhesions and for the transfascial sutures, or is it all bundled ?thanks
 
Lysis of adhesions is included. 44180 lap lysis will be denied as unbundling. Go with mod 22 if documentation indicates extra time spent (how much) and where the adhesions were.
